If you’re trying out for Dance Company for the upcoming school year (2016-2017), you’ll need to fill out and turn in a clearance packet. We’ve got some good news for you – the process is now online! Instructions and procedures are posted here on the school website.

In summary, there are two online forms students must complete (via FamilyID):

  • Yorba Linda 2016-17 Athletic Registration
  • DMV/Risk Management School Driver Registration form 2016-17

Please note:  your registration for the 2016-17 school year is not complete until you have also turned in the following two (2) documents to the athletic clearance clerk (Mrs. Hood) in the front office by Wednesday, May 18th (please do not turn any paperwork in to your coach):

  • Copy of front and back of your health insurance card
  • Physical – must be original and must be completed on district form
