All YLHS students who plan to audition for Dance Company must fill out a modified clearance packet and have a completed physical (signed by the doctor) turned into the high school office in order to try-out.  Returners are fine and can use the one from earlier this year. Remember that you will not be allowed to try-out until the completed packet is returned to Jacquelyn Roberts in the front office. DO NOT turn in your clearance packet or physical to Ms. Maes. Remember to turn in the original physical signed by the doctor. No copies can be accepted.

Physicals will also be held in the YLHS big gymnasium on Wednesday, May 28, 2014 from 4:00pm – 8:00pm (doors close at 7:00pm). The cost is $30.00. Please make checks payable to YLHS.
